TRAILS COVID-19 Safety Intake Form

HERE’S WHAT WE ARE DOING TO KEEP YOU SAFE:

We look forward to having you join our rehearsal and performance space and we are dedicated to your safety. In order to ensure the safety of all of our guests in this creative space, it is essential we follow a rigorous protocol.

We are working with all Actors’ Equity Association, Broadway League and Society of Stage Directors and Choreographers regulations in order to adopt best practices for a safe and organized return-to-work policy. This policy incorporates the developing guidelines set by the city and state on how to safely and effectively operate.

PROOF OF VACCINATION:

In accordance with New York City’s new regulations, TRAILS and Ripley-Grier Studios require either proof of vaccination for entry into the studios (12 years or older) or a negative PCR (polymerase chain reaction) COVID test taken within the past 72 hours for entry. We will not be accepting any rapid-antigen tests at this time.

VACCINATED INDIVIDUALS:

Be prepared to show Government-Issued Photo ID and your proof of vaccination in one of these forms:

  • physical vaccination card, photo of vaccination card, or the Excelsior Pass App/NYC COVID SAFE app

  • U.S. vaccinations – BioNTech / Pfizer, Moderna, Johnson & Johnson (may appear as Jansen or Johnson 2 or J2) 

  • International: AstraZeneca, Covishield, Covaxin,(India), Sinopharm (Beijing), Sinovac/Coronovac, Vero Cell/Comirnaty

  • Vaccines that are not allowed: Sputnik (Russian) and Cansino (Beijing) as they are not approved by the WHO

*All members of the cast and creative team must be fully vaccinated as defined by the CDC (two weeks after the last dose) to participate in the reading. They must wear proper masks at all times unless actively rehearsing or performing.

UN-VACCINATED AUDIENCE MEMBERS (under 12 years old):

Be prepared to show Government-Issued Photo ID and provide proof of negative PCR (polymerase chain reaction) COVID test taken within 72 hours of entry to the studios.

  • Test results in hard copy or on the phone.

  • At-home tests are not permitted.

*Anyone demonstrating a temperature of 100.4 degrees or higher will not be permitted to enter the facility. All audience members and creative team members must wear properly fitted face masks at all times; no exceptions.

FACE COVERINGS:

Masks covering the mouth and nose are required to enter the studios and while in all public areas, regardless of vaccination status. These face masks must be cloth or medical grade covering your nose and mouth at all times. We do not accept neck gaiters, bandanas, plastic masks, mesh masks, vented masks or ill-fitting masks. We will have masks to provide for you and will ask you swap to keep everyone in production safe.

COMMON AREAS:

Face covering must be worn in all common areas and social distancing rules must be followed to the best of your ability.

Restrooms: We have installed reminders for everyone to wash their hands thoroughly.

UPON ENTERING THE REHEARSAL AND/OR PERFORMANCE SPACE PLEASE HAVE YOUR PROOF OF VACCINE AND GOVERNMENT-ISSUED PHOTO ID IN HAND READY TO BE CHECKED.
